Can you drink too much? Over-hydration and hyponatraemia
Yes, though it is uncommon, and the fix for one problem can cause the other.
Exercise-associated hyponatraemia happens when someone drinks more fluid than they lose, diluting blood sodium below a safe level (under 135 mmol/L). A 2017 expert consensus led by Hew-Butler describes it, and the catch is cruel: early symptoms (headache, nausea, bloating, feeling off) resemble dehydration, so people drink more and make it worse.

Treat any article that gives you a precise perimenopausal litre count with suspicion.
Symptom overlap muddies the research
Because mild-dehydration symptoms (headache, fatigue, low mood, poor focus) overlap so closely with ordinary menopause symptoms, it is genuinely hard to isolate how much fluid alone contributes. Much of the mood-and-hydration evidence, including Armstrong’s 2012 work, comes from younger women in controlled settings rather than perimenopausal women in real life, so it is suggestive rather than definitive for this group.
The HRT and fluid-balance picture is individual
The link between oestrogen, sodium handling and fluid retention is well described in general physiology, but individual responses to HRT vary widely, and there is no clean rule for how it should change your drinking. This stays firmly in the discuss-with-your-clinician category rather than the settled-science one.
Electrolyte supplementation for everyday menopause is unproven
The case for electrolytes in long, hot, sweaty exercise and in illness is solid. The case for a daily electrolyte sachet to manage ordinary menopause symptoms is not, and current marketing runs well ahead of the evidence.
When to speak to your doctor
Hydration is usually a self-managed habit, but fluid balance occasionally signals something that needs a clinician. Treat the following as reasons to get checked rather than to drink more water and hope.
Red flags worth a conversation
- Constant, unquenchable thirst. The NHS advises always seeing your doctor if you feel thirsty all the time, because persistent excessive thirst can point to conditions such as diabetes, anaemia or a medication effect and should be investigated rather than simply drunk away.
- Passing a lot more urine than usual, especially alongside constant thirst and tiredness, which the NHS lists among the symptoms of type 2 diabetes and which warrants a check.
- Swelling (oedema) in your ankles, legs, hands or face, or bloating that is persistent rather than passing, which is worth raising rather than self-treating with fluid changes.
- Dizziness or light-headedness that does not settle, particularly on standing, which the NHS flags among warning signs of dehydration but which can also reflect blood-pressure or other issues.
- You take medication that affects fluid balance, such as diuretics, or you have a heart, kidney or blood-pressure condition; your fluid and electrolyte needs may be individual and should be guided by your clinician, not a generic article.
- Suspected hyponatraemia, meaning confusion, persistent nausea, severe headache or feeling very unwell after drinking large volumes of fluid, which is a medical situation, not a drink-more one.
When it is more urgent
If you think you are dehydrated and are not improving after a day, contact your doctor or a nurse advice line, or visit an urgent care clinic. Treat signs of shock, such as pale, blotchy or clammy skin, a racing heart, or feeling faint, as an emergency and call 911 or go to your nearest emergency room. If you or someone else becomes very drowsy, confused or unresponsive, call 911 rather than waiting.
None of this is meant to make a normal glass of water feel fraught. It is simply the line between a habit you can run yourself and a symptom that deserves a professional eye.

And “drink before you feel thirsty” is genuinely risky advice at the extreme, because forcing fluid past need is exactly how exercise-associated hyponatraemia happens, per the Hew-Butler consensus. For most days, most people, drinking to thirst and to urine colour is both safer and simpler.
The better mental model is to match hydration to your own symptom picture rather than a universal number:
- If your weeks are night-sweat-heavy: you are losing more fluid overnight, so weight your intake toward morning and daytime rehydration, keep water by the bed, and consider electrolytes only when the sweats are drenching. Do not gulp litres at 3am and wreck the rest of your sleep.
- If your weeks are exercise-heavy: learn your own sweat rate with a before-and-after weigh-in once, then replace deliberately around sessions, using the ACSM 2-per-cent guideline as your ceiling. This is where fluid actually changes how you feel and perform.
- If your weeks involve travel or heat: dry cabins and hot climates raise losses you will not always feel, so pre-empt with a bit more and go lighter on alcohol.

How much water should a woman over 40 drink a day?
Aim for roughly 2 to 2.5 litres of total fluid, which matches the EFSA adequate intake of about 2.0 litres of water for women plus the water in food. The NHS frames this as 6 to 8 glasses of fluid a day. Nudge higher if you sweat a lot from hot flushes or exercise, and use urine colour as your check.
Does perimenopause make you more dehydrated?
It can, mainly because night sweats and hot flushes increase fluid loss, and because thirst becomes a weaker signal with age. No authority has set a specific higher target for perimenopausal women, so the sensible approach is to replace what you lose and keep a light daily routine rather than wait to feel thirsty.
Is 3 or 4 litres of water a day too much in menopause?
For most women, that is more than needed, and pushing very high volumes carries a small risk of diluting blood sodium (hyponatraemia), described in the 2017 Hew-Butler consensus. Do coffee and tea count towards hydration?

What are the signs of dehydration in perimenopause?
Dark or strong-smelling urine is the most practical early sign, per NHS guidance. Headache, fatigue, poor concentration and low mood can also appear, as Armstrong’s 2012 study found in mildly dehydrated women. These overlap with ordinary menopause symptoms, so treat hydration as one thing to rule out, not the whole answer.
Do I need electrolyte drinks during menopause?
Usually not for everyday life. According to the ACSM, sodium and chloride are the main electrolytes lost in sweat, so electrolytes earn their place during long or very sweaty exercise, drenching night sweats, hot weather or illness with vomiting or diarrhoea. On a normal day, plain water and a varied diet cover it.
Can drinking more water help with hot flushes?
It supports your body’s heat-shedding responses and replaces the fluid lost when you sweat, so staying topped up helps you cope with flushes. It is not a proven treatment that reduces how often they happen, so think of it as a helpful foundation rather than a cure.
Does water help with menopause belly bloat or HRT bloating?
Restricting water does not fix bloating and can backfire. Oestrogen influences water retention, which is why some women bloat when starting HRT, an effect clinicians describe as usually temporary. Adequate fluid plus potassium-rich foods supports normal fluid balance; persistent bloating is worth raising with your prescriber.
Should I drink water before I feel thirsty?

How do I know if I am drinking enough?
Check the output rather than counting the input: pale-straw urine for most of the day is the everyday green light the NHS points to. If it is consistently dark, or you feel headachy and foggy, add fluid and reassess.
Is it worth tracking my water in an app?
For some people it builds the habit, especially early on. But the simpler tool is urine colour plus meal-anchored glasses. Track if it helps you, drop it once the routine sticks, and do not let an app number push you into forcing fluid you do not need.
